The HRA was first conducted in 1998/1999 by The University at Buffalo Department of Family Medicine Research Institute with funding from The Public Health Coalition as a means of acquiring health behavior data at a county-specific level. The report was distributed throughout the community. A few examples of it's use were preparation of hospital community services plans, community health assessments, the State of the Region Report and a presentation at Healthy People 2010.

In 2004/2005 the study was conducted by Health For All. Funding was provided by The Community Health Foundation of Western and Central New York and a HRSA Community Access Program. Many questions remained the same allowing us to compare trends over time. Some questions were improved or removed. New questions were added. The final questionnaire can be downloaded here. Households in the 8 counties of Western New York were called using a random digit dialing system. Eligible participants were asked a series of questions about themselves and others in the household. Participation was completely voluntary and no information was collected on individuals under 18 years of age.

Once collected, the data was analyzed by the University at Buffalo's Department of Social and Preventive Medicine and Population Health Observatory.
